Changing your contact details
To update details like your name, address, phone numbers or email address
- Visit the surgery from 8am to 6pm and complete a change of details form.
- We are unable to accept any changes over the telephone
Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.
Changing your phone number or email address
You will need to show us identification before we can change your phone number or email address.
Changing your name or date of birth
If you need to update your name or date of birth on our records, please bring in one of the following into the surgery and complete a form.
Name change – Marriage certificate, deed poll, or other official document showing new name
Date of birth -Birth certificate or passport
We are unable to update these details without seeing proof, to keep your medical record accurate and secure
